AVIATION MEDICAL SERVICES

EASA Class 1

Dr Fonso is authorised by the UK CAA to perform REVALIDATION and RENEWAL EASA Class 1 medicals.

NB: initial Class 1 medicals can only be performed at an Aeromedical Centre (AeMC).

What to bring to your medical:

  • Photographic ID (passport or UK Photocard Driving Licence)

  • A completed application form: CAA Med160

  • Your most recent medical certificate (if applicable)

  • Your glasses or contact lenses (if applicable)

  • An optician’s report on CAA form Med 162: CAA Med162

    • A report is required no less frequently than every 2 years if you wear glasses or contact lenses and/or if you have a VDL or VNL limitation on your medical certificate.

EASA Class 2 and LAPL

Dr Fonso is authorised by the UK CAA to perform INITIAL, REVALIDATION and RENEWAL EASA Class 2 (Private Pilots Licence – PPL) and LAPL (Light Aircraft Pilot Licence).

What to bring to your medical:.

  • Photographic ID (passport or UK Photocard Driving Licence)

  • A completed application form: CAA Med160 for Class 2 applicants or LAPL Form for LAPL applicants.

  • Your most recent medical certificate (if applicable)

  • Your glasses or contact lenses (if applicable)

  • A recent (less than 2 years old) spectacle prescription (if applicable) at your initial medical

EASA Class 3 (ATCO Medical)

Dr Fonso is authorised by the UK CAA to perform REVALIDATION and RENEWAL EASA Class 3 (Air Traffic Control Officer – ATCO) medicals.

What to bring to your medical:.

  • Photographic ID (passport or UK Photocard Driving Licence)

  • A completed application form: EU3 Application Form

  • Your most recent medical certificate (if applicable)

  • Your glasses or contact lenses (if applicable)

  • An optician’s report on CAA form EU3 Comprehensive Eye Examination Form: CAA EU3 Eye Form

    • A report is required no less frequently than every 2 years if you wear glasses or contact lenses and/or if you have a VDL or VNL limitation on your medical certificate.

Cabin Crew

All new cabin crew require and initial medical examination. Periodic cabin crew assessments are required for all cabin crew at intervals of no more than 60 months. More frequent medical examinations or assessments may be required in certain circumstances, such as the crew member having a medical condition requiring regular surveillance.

  • Photographic ID (passport or UK Photocard Driving Licence)

  • A completed questionnaire for INITIAL applicants: Initial Questionnaire

  • Or a completed questionnaire for RENEWAL applicants: Periodic Questionnaire

  • Your glasses or contact lenses (if applicable)
