Fees and Payments
Fees are payable on the day of the consultation. We accept most major credit cards, cash or cheques with a cheque guarantee card.
We charge 25% of the full fee if a cancellation occurs within 24hrs of the time of the consultation. A cancelled appointment that is rescheduled to a vacant slot on the same day will not attract a cancellation fee
Dr Fonso is registered with the Information Commissioner’s Office. We comply with the Data Protection Act and the Access to Medical Records Act and the information we hold on file for you will not be released to third parties without your consent. We run a paperless clinic and utilise sophisticated and heavily protected IT systems to hold your data.
Access To Health Records
You have the right to see and have access to your medical records and may request a copy of these at any time, subject to a small administrative charge. If you require your medical records please email us at email@example.com
You will always be asked whether you require a chaperone prior to any examination and we can have one available for you. You are also welcome to bring a family member or friend with you to your visit. Where possible, please indicate at the time of booking whether a formal chaperone is required. Where a chaperone is required but not available, your appointment will be rescheduled at a time that is convenient for you.
We hope that you never have reason to complain about our service, however, we recognise that things can go wrong. We have a robust complaints process that involves external and independent adjudication. If you have cause for concern or a complaint, please address it in writing to Dr Fonso at firstname.lastname@example.org in the first instance. Your complaint will be acknowledged within 2 working days.
